Assessments and Enrollment
After eligibility is determined, our skilled staff works with the individual and their family to assess their needs and the resources available. The assessment process matches interests and needs with program availability. The outcome of the assessment period is an Individualized Service Plan (ISP) that specifies what services will be delivered and how an individual's needs will be met through enrollment.
